We would hope that each visit to our office is like visiting with friends. With a gentle touch and a caring manner we offer you and each of our patients the best that modern dentistry has to offer. Dental treatment and care has never been expanding as fast as today and as your dental health team we constantly take advantage of ongoing training to make sure you receive the best proven care available.

Annette -- Administrative Coordinator

With her years of experience and knowledge in the dental field, Annette can answer any questions you may have regarding scheduling and dental procedures.

Deena -- Administrative Assistant

We have worked hard to make quality dental care affordable and Deena is our specialist in helping you with your insurance questions and billing.

Sharla -- Dental Assistant

Stephanie -- Dental Assistant

Tori -- Dental Assistant

Sharla, Tori and Stephanie have special training where they prepare many of the procedures and assist in your treatment while working to make your visit as comfortable as possible.

Anna-- Dental Hygienist (RDH)

Anna meets the needs of our patients to obtain and maintain their best in oral health. She focuses specifically on health of the gums through preventitive maintenance and oral hygiene instruction.

How to Find Us
Location/Map

Our office is located near West Valley Medical Center in Caldwell, Idaho, at 1916 Ellis Avenue.

From Nampa travel west on Karcher Road until you come to 10th Ave. Turn North (right, toward Caldwell) and travel approximately 4.5 miles to Elm Street (just before the West Valley Medical Center). Turn Left (West) on Elm, past the West Valley parking, to Ellis Ave. and turn Left. You can park immediately in front of our office.

From Homedale travel East on Homedale Road until you come to 10th Ave. Turn North (left, toward Caldwell) and travel approximately 2.5 miles to Elm Street (just before the West Valley Medical Center). Turn Left (West) on Elm, past the West Valley parking, to Ellis Ave. and turn Left. You can park immediately in front of our office.

From North of Caldwell travel South on I-84 to the 10th Ave. off ramp in Caldwell. Turn right onto 10th Ave. and continue through the Fairview Golf Course. Just beyond West Valley Medical Center turn right (west) on Elm, past the West Valley parking, to Ellis Ave. and turn Left. you can park immediately in front of our office.

Making an Appointment

To make an appointment simply call our office at 459-2376. Annette or Leigh will be happy to schedule a visit and answer many of the questions you may have. They can also help you with insurance information you may need to bring with you. If you have any medical conditions, special needs, or medication requirments please mention them to us dring your call. This will help us be prepared to treat your dental needs and make your visit a pleasant experience.

When you come to our office for the first time we will have you complete our Health History and Patient Information forms. These important documents help us give you the care your need and deserve.

If your visit is for an emergency, please call us immediately. We won't leave you hurting.

Monday -- Thursday
8:00 am -- 5:00pm

Fridays -- 8:00 am -- 12:00 pm

If you are calling for an appointment we will schedule a time most convenient for all of us. If you receive a recording during office hours it means all lines are busy at that time. Please leave us a message and we will return your call as soon as possible.

If you visit is for an emergency, please call us immediately. We won't leave you hurting.

Emergencies

Emergencies can happen to anyone. If you should have a dental emergency call our regular phone number,

immediately. During regular business hours Annette or Leigh will get you in to see either Dr. John Blaisdell or Dr. Tom Blaisdell as soon as possible, generally the same day.

If your emergency should happen after regular office hours or on the weekend call our same number. You will be instructed what to do. We won't leave your hurting!
